Patient Information

Once your GP has made the Specialist referral please call the rooms to arrange an appointment time.

We will see you as soon as possible.

For your first consultation please ensure you have:

  • Checked the location of consultation, arrive early to complete relevant forms
  • A copy of your GP referral
  • Hard copies of your images – eg XR, CT and MRI. The films are preferred to a CD
  • A list of medications
  • Medicare number, health insurance details, workcover documentation, DVA etc
  • Other medical documentation relevant to your condition
  • Payment at completion of consultation by Debit or Credit Card (Medicare will rebate part of fee)

For follow-up appointments please bring your films again for review.

We hope to see you within a few weeks but if you find yourself deteriorating beforehand please ring the rooms and we may be able to expedite your consultation.

If you are unable to attend your appointment please let the rooms know as soon as possible so that we can book another patient.
